How to Apply for SSD Benefits throughout California, Nevada and the West

What You Need to Know About Applying for SSD Benefits

There are two key parts to all SSD claims: the Disability Benefit Application and the Adult Disability Report. Both forms can be completed either on paper or on the Social Security Administration’s (SSA) website. Applying online saves time and money, and our attorneys can still prepare all of the necessary documentation on your behalf. If you do not wish to file online, you will need to schedule an appointment with your local Social Security office for an initial interview before you can start the application process.

The following list identifies the basic information you need in order to file an SSD claim in Oakland and Sacramento:
  • Your original birth certificate or proof of U.S. citizenship or lawful alien status;
  • Wage and salary records from the last year (W-2 or Form 1040 with appropriate Schedules);
  • Your employment history over the past 15 years;
  • Your minor children’s and spouse’s names and birthdays;
  • The date(s) on which you were married and divorced;
  • Military service discharge information (from your Form DD 214);
  • Contact information for the doctors who have treated the injury or illness that caused your disability;
  • All medical records relating to diagnosis and treatment of your disability;
  • A list of your current prescriptions; and,
  • Information on any insurance or workers’ compensation claims you have filed pertaining to your disability.
